Not sure what you mean by "well warmed up" account but I assume the accounts you're trying to run on don't have a QS as high as the "unicorn account".
Have you tried starting off with a higher budget, launching at midnight as per account timezone?

As you answered your question yourself I can only agree - before you submit a campaign review TOS. Create transparent/coherent ads that lead to a complying landing page.
FB isn't for the weak, if someone doesn't have the due diligence to make things work it's probably better just to try...

First of all I'd recommend you to create a FB page for your blog or website. This will be the page FB users will see when you run a Sponsored Newsfeed post.
If you're aiming just for lots of clicks to your website I'd set up a "Clicks to Website" objective...

Are you split testing several landing pages?
How many clicks did you get for $12?
What was your CTR on ads and LP?
Are you rotating several offers on your LP?
and so on...
I don't think $12 ad spend has generated...

I have to warn you about getting your account flagged pretty soon if you attempt running sweeps on FB. Just be prepared with backup accounts if you're serious about running BH.
TBH I'd focus on a WH offer instead of running offers that are against FB's T&C.
