After having been an affiliate for more programs than I can count over the last several years, I've have recently had the opportunity to design a program for a client of ours. We have been working steady over the last four months to build an affiliate program that we trust will be a terrific addition to anyone running small to medium size business oriented campaigns.
Company Description:

Affiliate Info & Signup:
Affiliate Partner Program
Program Type:
CPA / Incentives based on Applications submitted.
Incentive:
$75 per approved application or $10 per completed application.
Region:
U.S. (actually US Merchant Applications only)
Payment Method:
Checks only (ACH forthcoming)
Campaign Support:
You can specify optional campaign ids in affiliate URL. These campaign ids are reflected in system status notifcations, payment statements, and the affiliate dashboard.
Affiliate Notifications:
- Email to affiliate when application was submitted as a result of their affiliate ID.
- Email to affiliate when transaction was approved or denied.
Affiliate Control Panel (custom):
- View Number of Cookies set as a result of Affiliate ID.
- View the number of times a visitor landed on an application page.
- View the number of applications submitted.
- View conversion rates (percentage of approved applications by total submittals).
- View status for applications pending, approved, or denied.
- View account payment statements.
- A Link Builder Tool to assign campaign ids, set cookies on promo sites, etc.
Affiliate Recognition:
We operate about a dozen promotion sites, each with a ?recommended sites? section where we highlight various affiliates via banner and text links. Cookies set on main site, or promo sites are honored throughout the network.
Promotional Products:
We are developing a line of Free mobile phone credit card processing applications that support Authorize.Net and PayPal Web Payments Pro. Currently the BB version is available for free download, Android version in Beta now, Pre on the way, iPhone available, but not as a free download. So far it has been a terrific tool to get potential customers to the site.
We also manage about a dozen mini-promotion sites that contain teaser forms. Affiliates can direct visitors to the network sites where cookies are set and honored throughout the network. This is true for all but one of our 13 promo sites.
Customer Support Services (complimentary):
- Affiliate Account Representative with toll free contact number
- Design resources for custom created banners, graphics, etc.
- Writers for creating custom content, i.e. blogs, news, etc.
- Creation of custom "Teaser" or "Pre-forms" for affiliate sites.
Affiliate Performance & Conversion Rates:
Top Affiliate Dec (so far): 45 applications
Highest Number of cookies set by an affiliate (one month): 636
Conversions based on applications submitted vs. approved transactions.
Highest ?Per Approved Application? conversion ($75): 40.0%
Lowest ?Per Approved Application? conversion ($75): 12.1%
Highest ?Per Approved Lead? conversion ($10): 100%
Lowest ?Per Approved Lead? conversion ($10): 50%
If you are looking for estimates regarding per click / per approved transaction, I don't have a standard number available, although I suppose it could be derived from the info above.
For reference, the highest conversions originate from sites that cater to business owners.
