How to apply to Affiliate networks

Hi K, your posts are great and very helpful for newbies like me.

However I do have some questions regarding the website in applications. Is this very important for application? If I apply with no website, would it be affecting the approval? I am acting as an Individual, not a company, hence I do not know what kind of website I have to create. If its purpose is just to have an online identity, is it more proper to submit them my facebook profile or my personal identification document?
