Mate, have you tried implementing it?

There are no fields inside because you'll grab the form fields (including the button) from AWeber or other email service you use(they generate it for you). You can style the button inside the email service if you want.

Not yet, I will give it a try soon, but there is an image in the folder that says apply now and I guess I can setup my aweber form without a button and use that one.
