Seeking Help Which documents to submit for Google Ads advertiser verification as affiliate marketer?

Can someone help me to which documents to submit while filling Google ads business operations verification form? As once I've already tried and failed and now my account is at risk, so I need urgent help. I'm confused about these points:

1: "Your Company Name" I don't have registered business, so can I use my website name as company name or need to use my own name.
2: "Please provide the name of the affiliate and their website. I've hundreds of affiliate partners through different affiliate networks and not sure that whether I should add here network name or affiliate network name and how to fill this section.
3: "Upload any document that support your sections above", I'm confused whether I should submit service agreement with affiliate partner or affiliate network. Also which kind of document is acceptable like screenshot or only download file and where to get these documents.
4: Under section 28 this is the most confusing "Attach any additional documents that supports the claims made above." Kindly guide me which documents I can submit here and from where I can get these documents: "Proof of all relationships" & "Other documents related to business operations and advertising agencies".

  1. You are an "agent" not a principal? An affiliate and not the one fulfilling the product offered is what that means, or; an agency for a client that pays you a percentage of the ad spend?
  2. You can do business in your own personal name (theoretically) but your domain name is not a legal entity --domain names are your personal property paid for under an agreement with a registry.
  3. Looks like Google may want you to disclose your personal name and address on the TOS. this is why people in business use registered assumed names and mail drops --because you are not a substantial business.

I'm operating an affiliate website where I offers coupons, discount deals and publish blogs for online stores and brands and earns commission when customers buy through affiliate links on my site. Also run Google ads campaign for some stores at my own cost.
