Plain, templates get screwed up in certain browsers, also it's better to email your list as you would email a friend. You wouldn't send a cool high graphic template email to your friends 99% of the times. Keep it plain, simple and to the point.
I've tried templates vs plain a lot of times and plain wins hands down EVERY time.

I voted for template, but I have to say that I use both. For my newsletters and special mailings, I use templates that are part of my branding. For everything else I generally use plain text.
Neither. Whitelist instructions instead of the redundant thank you page after signup. Templates work just fine if they're responsive. Your template tests are probably failing because they probably look like crap in mobile and desktop email clients.
